In fact there are no academic specializations for different types of buildings, such as theatre specialist or hotel specialist. We say an architect is trained to design all types of buildings. The pre-design study about a particular project and the analytical approach is all that important. However, often architects are labelled as specialists by the people.
Suppose your uncle commissioned you to design his nursing home and you it successfully well. It is quite likely that his friend who also is a doctor appoints you to design his hospital. Then being in the same profession others also come to know about your work, and call on you. Saying that you are experienced in designing hospitals and in that sense people call you a hospital specialist.
